Columbia Care Services is seeking a Housing Specialist I to support individuals with behavioral health needs in achieving housing stability and recovery. In this role, you will provide housing-focused case management, assess needs and barriers, and develop individualized housing plans. You will help clients identify, apply for, and secure safe, affordable housing; coordinate with landlords, housing authorities, and community partners; and connect clients to benefits, healthcare, and employment resources.
Working in a client-first, trauma-informed, and collaborative environment, you will maintain accurate documentation, participate in team meetings, and promote recovery, independence, and community integration.
Responsibilities
- Provide housing-focused case management to clients with behavioral health needs
- Assess housing needs, strengths, and barriers; develop individualized housing plans
- Assist clients in locating, applying for, and securing safe, affordable housing
- Coordinate with landlords, housing authorities, and community partners to support placement and stability
- Educate clients on tenant rights, responsibilities, and independent living skills
- Link clients to benefits, healthcare, employment, and community resources
- Maintain accurate, timely documentation in accordance with agency and regulatory standards
- Participate in team meetings, case consultations, and quality improvement activities
- Apply trauma-informed, recovery-oriented practices in all client interactions
- Support crisis prevention and de-escalation and follow safety protocols
