RN/LPN Pediatric Homecare Nurse – Family First Homecare
Family First Homecare (FFHC) is currently seeking compassionate and skilled RNs and LPNs to provide one-on-one in-home nursing care for multiple pediatric and young adult patients in the area. We have several opportunities available, but we are specifically looking for a confident, experienced nurse for a 17-year-old female patient receiving both hospice care and in-home nursing support. The ideal nurse for this case should be comfortable managing care independently before entering the home.
The family and patient are supportive, though the home environment can be busy.
Available shifts for this patient:
- Sunday 7AM–7PM
- Monday 7AM–7PM
- Saturday 3PM–7PM
- PRN coverage available, primarily nights 7PM–7AM, though day nurses may also be utilized as needed
Patient care needs include:
- G-tube care
- Oxygen support
- Breathing treatments
- Catheterization
- Hospice-supportive care
- Monitoring comfort, safety, and daily care needs

Requirements:
- Active RN or LPN license
- Minimum 1 year of nursing experience
- Current CPR certification with hands-on skills check
- Clear background screening
- Valid driver's license and proof of insurance, where applicable
- TB and Hep B screening, where applicable
- Ability to communicate effectively in English

Key responsibilities:
- Accountable for providing care that is based on licensure/certification within the scope of practice.
- Performs admission, transfer, re-certification, resumption of care and discharge paperwork as assigned.
- Maintains an updated clinical record on each patient at all times, meeting required deadlines for documentation.
- Evaluates the effectiveness of nursing service to the patient and family on an ongoing basis.
- Consults with physicians concerning alterations of patient care plans, documentation of changes in patient care and reports all changes in care to the nurse supervisor.
- Supports FFHC's mission by mentoring and/or coaching new employees during patient specific orientations.
- Educates patients and their caregivers on topics related to patients' care needs.
- Performs supervision for LPNs and Aides in the home as assigned.
- Participates in FFHC Quality Assurance Performance Improvement program (QAPI) as requested.
- Attends and/or completes all mandatory training/ in-services as assigned.
- Submits required personnel file documentation in a timely manner.
- Adheres to FFHC compliance and ethics expectations.
- Maintains patient and family confidentiality.
- Maintains professional, supportive, and responsive interpersonal communication skills.
- Assesses physical, functional, psychosocial, and cognitive status of the home care patient utilizing interview observations and physical exam techniques.
- Assesses the home environment for safety and infection control.
- Incorporates multidisciplinary data into the nursing assessment of the home care patient.
- Provides ongoing nursing assessment with the patient, family, and home environment to determine physiologic or psychosocial risk. Applies previous nursing experience and base of knowledge and adapts with the patient and family in a home setting.
- Assesses for the presence of advanced directives and facilitates further action in obtaining information about or implementing advanced directives if indicated.
