Registered Nurse

Town Square Hospice is dedicated to providing dignified and compassionate care to patients and families navigating life-limiting illnesses. Our mission focuses on enhancing quality of life by addressing physical, emotional, and spiritual needs. Through an interdisciplinary team of healthcare professionals and volunteers, we deliver personalized, holistic care in patients' homes, senior communities, or long-term care facilities. We also extend resources to families, offering bereavement support for up to 13 months after their loss.

Available 24/7, we walk beside families with comfort and guidance, honoring life at every stage.

This is a PRN, on-site role for a Registered Nurse located in Medina, OH. As a Registered Nurse, you will provide compassionate clinical care for hospice patients, implement personalized care plans, monitor patient health, and manage pain and symptom relief. You will collaborate with physicians, social workers, and other interdisciplinary team members. Additionally, you will ensure diligent documentation, educate families on patient care, and provide emotional support to patients and their loved ones throughout their journey.

• Strong clinical nursing skills, including patient assessment, symptom management, and administering medications
• Experience in end-of-life care and pain management
• Effective communication and collaboration capabilities with patients, families, and interdisciplinary teams
• Ability to create and manage personalized care plans
• Strong organizational and time management skills to ensure proper documentation and task prioritization
• Registered Nurse (RN) license in the state of Ohio
• Current Basic Life Support (BLS) certification
• Experience in hospice or palliative care nursing is preferred but not mandatory
• Compassionate approach to patient care and emotional support
• Flexibility to adapt to varying patient needs and work in diverse care settings
