Role Description

This full‑time, on‑site Registered Nurse role is based in Medina, OH. The Registered Nurse will provide direct skilled nursing care in clients’ homes, including assessments, medication administration, wound care, and monitoring of chronic and acute conditions. Daily tasks include developing and updating care plans, coordinating with physicians and therapists, educating clients and family members on treatment and self‑care, and documenting care in accordance with clinical and regulatory standards.

The role also involves collaborating with caregivers and other members of the care team to ensure safe, high‑quality, and person‑centered services consistent with HIPAA and organizational policies.

Qualifications
  • Active Registered Nurse (RN) license in the state of Ohio and current BLS/CPR certification.
  • Clinical skills in assessment, care planning, medication administration, wound care, and management of chronic and post‑acute conditions.
  • Experience in home health, senior care, or related settings, including Alzheimer’s/dementia and personal care support.
  • Strong communication, documentation, and interpersonal skills for working with clients, families, and interdisciplinary care teams.
  • Ability to work independently in the field, manage time effectively, and travel locally to client homes.
  • Knowledge of HIPAA, infection control, and relevant home health regulations and best practices.
  • Comfort with using electronic health records or digital documentation tools.
  • Flexibility to work varied schedules as needed and a valid driver’s license with reliable transportation.
