POSITION SUMMARY

The Hospice Clinical Social Worker, Master of Social Work (MSW) coordinates the psychosocial care and services for assigned patients in collaboration with the patient, family, physician, patient care team, and payers. The Social Worker utilizes advanced psychosocial skills to facilitate the coordination of care through assessment, planning, intervention and evaluation. The Social Worker promotes and evaluates the effective utilization of resources using current knowledge, awareness of community services, and serves as a role model to achieve optimal clinical and resource outcomes.

Job Requirements Primary Accountabilities
  • Conduct psychosocial assessment to screen patients/families at risk for crisis, abuse, financial or environmental issues, collaborating with RN Case Manager, ancillary staff and licensed practitioners to facilitate continuity of care, ensuring mandatory reporting processes are completed as required by regulations and best practices.
  • Assess and document in EMR patient and family needs, work with interdisciplinary team to identify psychosocial needs.
  • Provide therapeutic or crisis intervention counseling with patients and families, supportive services for end‑of‑life issues, terminal care and bereavement, counseling and research placement options depending on patient needs, and deliver therapeutic interventions as needed.
  • Document professionally, accurately, timely and according to Hospice of Health First policies and procedures, noting transitional care needs (LTC facility, respite, and transfer to inpatient unit).
  • Communicate with patient, caregiver and interdisciplinary team regarding payer or financial issues, ensuring comprehension and awareness of payer/reimbursement practices and regulations affecting the patient’s plan of care.
  • Share pertinent patient information and plan of care effectively with appropriate team members; collaboratively determine desired end‑of‑life goals and communicate options to patient/family, physicians and staff.
  • Identify “at risk” patient populations, strengths of support systems, and identify structure or process breakdowns and improvement opportunities; distinguish risk management and quality issues and intervene or refer as indicated.
  • Intervene and recommend appropriate referrals to resolve barriers to patient progression, collaborating with other disciplines as directed, initiating referrals to Ethics Committee, Risk Management or legal services as appropriate.
  • Ensure compliance with regulatory standards of care at state and federal levels and verify compliance with National Patient Safety Goals.
  • Recommend supportive and therapeutic interventions as indicated to ensure a cost‑effective, safe and efficient plan of care and quality patient outcomes.
  • Reinforce and document education with patient/family on psychosocial aspects of care and expected outcomes, and provide and note education for team members regarding appropriate services and referrals available for patients and families.
  • Participate in resolution of legal issues such as guardianship, power of attorney, etc., and assist with the implementation of advanced directives.
Minimum Qualifications
  • Education:

    Master’s Degree in Social Work (MSW).
  • Work Experience:

    None.
  • Licensure:
    Driver’s License and clean Motor Vehicle Report (MVR) maintained.
  • Certification:
    American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card prior to start date and maintained.
  • Background Check:
    Satisfies the State of Florida Agency for Healthcare Administration (ACHA) Level II background screening.
  • Skills/Knowledge/Abilities:
    • Proficient in Microsoft Office – Teams, Outlook, Word, etc.
    • Demonstrated ability to work in various healthcare systems (e.g., Epic EHR/EMR).
    • Ability to handle multiple demands in a short timeframe, meet deadlines, and be a self‑starter.
    • Committed to quality patient care and maintaining the professional standards of Social Work.
    • Ability to work in a team environment effectively.
    • Ability to relate to and work with the ill, elderly and emotional individuals within hospice and community settings with compassion and professionalism.
