Home Care Nurse Manager - Health First Home Care

Position Summary

To be fully engaged in providing No Harm/ Quality care, Customer Experience, and Stewardship through the coordination, implementation, and optimization of appropriate, patient focused care while ensuring clinical, customer/patient satisfaction, regulatory and financial goals are being met. The manager is responsible for supervision of supervisory and clinical staff, the intake department, and clinical liaisons, while supporting the IDN in a collaborative relationship with our hospital leaders, medical group and community partners.

• Lead daily clinical operations to ensure quality patient care, appropriate utilization, regulatory requirements, and customer/patient satisfaction.
• Demonstrate and promote strategic thinking and drive problem‑solving by empowering and supporting clinicians to assume responsibility and accountability within their scope of practice.
• Maintain and disseminate best practice clinical information obtained from across Health First and from external resources.
• Provide performance tools and a system of accountability by proactively monitoring leading indicators for patient safety, comparing performance outcomes against top decile benchmarks, and ensuring action planning to continuously improve patient outcomes and customer experience.
• Oversee and manage the development of appropriate policies and procedures to ensure compliance with the necessary regulatory agencies including AHCA, The Joint Commission, etc. Monitor and ensure compliances with resource allocation.
• Direct quality and efficient care coordination to ensure an optimal member/patient experience and maximize customer satisfaction.
• Lead business development initiatives for strategies to build referral volume by managing hospital and non‑hospital liaison team to achieve financial benchmarks.
• Manage the clinical intake referral team by ensuring the safety and appropriateness of patient acceptance and facilitate timely initiation of care with proper disciplinary team members.
• Serves as an IDN partner by mediating and assisting with complex problem solving, conflict resolution, and prioritization utilizing continuous improvement principles.

• Define and communicate a clear, compelling vision for the team that effectively ties into the mission and vision of Health First, and inspirationally lead the team to achieve that vision.
• Provide leadership, motivation, coaching, feedback and support to foster and strengthen growth and development of an effective, high performing team.
• Lead change through effective communication, explaining the connection and value to the organization, creating stronger buy‑in and urgency, while understanding impact to the team to obtain commitment.
• Demonstrate openness to hearing diverse ideas and thoughts; create a sense of inclusivity; and encourage collaboration across teams to help break down silos to meet the team’s and organization’s goals.

• Education:
  
  BSN
• Work Experience:
  
  Three (3) years Healthcare leadership experience.
• Licensure:
  • Current RN Licensure in the State of Florida or endorsement.
  • Valid Driver’s License and clean Motor Vehicle Record and maintained.
  • Certification:
    American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider Completion Card prior to start date and maintained.
  • Background Check:
    Must satisfy Florida of Agency for Health Care Administration (AHCA) Level 2 background screening.

• Work Experience:

• Majority of time involves sitting or standing; occasional walking, bending, and stooping.
