Position SummaryAssess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and correct disabling conditions resulting from disease or injury.
ResponsibilitiesPerform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Apply concepts of infection control/universal precautions in performing patient care.
Assure continuity of quality patient care delivered with appropriate documentation.
Provide effective communication to patient/family, team members and other health care professionals.
Promote agency philosophy and administrative policies.
Maintain and complete all documentation within company‑stated guidelines.
RequirementsSuccessful completion of an accredited physical therapy program and state licensure exam.
Valid Physical Therapy license.
One year of home health experience preferred.
CPR certification.
Valid driver's license.
Valid auto insurance.
Home Care Home Base experience preferred.
Physical DemandsMust be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
Must be able to lift, carry, pull, or push up to 100 lbs.
