Position: OCCUPATIONAL THERAPIST

HCA Healthcare is seeking a dedicated Home Health Occupational Therapist to deliver high-quality, patient-centered care in the comfort of patients' homes. In this role, you will assess functional abilities, develop individualized treatment plans, and implement therapeutic interventions to improve independence and quality of life. You will collaborate with an interdisciplinary healthcare team, educate patients and families, and document care in compliance with regulatory standards.

Responsibilities

  • Provide comprehensive occupational therapy evaluations in the home, assessing functional status, home safety, and patient goals.
  • Develop, implement, and regularly update individualized, evidence-based plans of care focused on improving independence and safety in daily activities.
  • Deliver therapeutic interventions including ADL and IADL training, functional mobility and transfer training, upper extremity strengthening, and energy conservation techniques.
  • Conduct home safety and fall-risk assessments, recommending adaptive equipment, assistive devices, and environmental modifications as needed.
  • Educate and train patients, families, and caregivers on safe performance of daily activities, equipment use, and strategies to manage functional limitations.
  • Collaborate and communicate regularly with nurses, physical therapists, physicians, and other team members to coordinate and optimize patient care.
  • Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and promptly in the electronic medical record in accordance with regulatory and payer standards.
  • Manage a home health caseload and daily schedule independently, planning efficient home visits while maintaining high-quality patient care and professionalism.
  • Adhere to infection control, safety protocols, and ethical standards in all home visits and patient interactions.
  • Participate in case conferences, staff meetings, and ongoing education or quality improvement initiatives to support best practices and professional growth.
