Social Worker LCSW
The Social Worker LCSW is fully engaged in providing quality/no harm, customer service and stewardship by utilizing advanced psychosocial skills to facilitate the coordination of patient care through assessment, planning, intervention and evaluation. The Social Worker LCSW uses current knowledge to effectively evaluate, promote and utilize hospital and community resources and services to achieve optimal outcomes, serving as a role model for coordination of psychosocial services for assigned patients in collaboration with the patient, family, physician, patient care team, and payers.

- Education:
Master's of Social Work - Licensure:
Current Florida LCSW license - Certification:
Current American Heart Association Basic Life Support Healthcare Provider Completion Card upon hire and maintained - Work Experience:
One (1) year of previous social work experience - Knowledge/Skills/Abilities:
Preferred Qualifications
- Certification: CCM or ACM Case Management Certification
- Work Experience:
Case Management experience in Hospital setting
Physical Requirements (Sedentary)
- Majority of time involves sitting or standing; occasional walking, bending, stooping
- Long periods of computer time or at workstation
- Light work that may include lifting or moving objects up to 20 pounds with or without assistance
- May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise
- Communicating with others to exchange information
- Visual acuity and hand-eye coordination to perform tasks
- Workspace may vary from open to confined
- May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle
