This position is based in Melbourne, FL and requires daily travel within Brevard County.

Monday-Friday, 8:00 AM - 5:00 PM.

Salary Range: $85, annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

You will then continue your onboarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

• Coordinate admissions of hospice patients by making skilled assessments of patients’ initial physical, psychosocial and spiritual needs.
• Coordinate the establishment of the initial plan of care with the attending physician, medical director and the inter-disciplinary team.
• Provide skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of the physician.
• Function as a liaison between disciplines and facilities during the admission process.
• Develop, prepare and maintain individualized patient care progress records with accuracy, timeliness and according to policies. Submit accurate documentation per care center policy. Record pain/symptom management changes/outcomes as appropriate.
• Communicate significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Obtain physician orders as required.
• Perform on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

• Current RN license, specific to the state(s) you are assigned to work.
• 2+ years of clinical RN experience in acute healthcare setting(s)
• Current CPR certification.
• Valid driver s license, reliable transportation, and liability insurance.

Preferred:

• Hospice or home health experience
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities
• Proficient in electronic medical record (EMR) systems
