This position is based in Melbourne, FL and requires daily travel within Brevard County.

Monday-Friday, 8:00 AM - 5:00 PM.

Salary Range: $85, annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

Orientation, Onboarding & TrainingYou will then continue your onboarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.
Responsibilities- Coordinate admissions of hospice patients by making skilled assessments of patients’ initial physical, psychosocial and spiritual needs.
- Coordinate the establishment of the initial plan of care with the attending physician, medical director and the inter-disciplinary team.
- Provide skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of the physician.
- Function as a liaison between disciplines and facilities during the admission process.
- Develop, prepare and maintain individualized patient care progress records with accuracy, timeliness and according to policies. Submit accurate documentation per care center policy. Record pain/symptom management changes/outcomes as appropriate.
- Communicate significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Obtain physician orders as required.
- Perform on-call responsibilities and on-call services to patients/families as assigned.
- Performs other duties as assigned.
- Current RN license, specific to the state(s) you are assigned to work.
- 2+ years of clinical RN experience in acute healthcare setting(s)
- Current CPR certification.
- Valid driver s license, reliable transportation, and liability insurance.
Preferred:
- Hospice or home health experience
- Strong communication and interpersonal skills
- Ability to work independently and manage multiple priorities
- Comfortable with daily travel between facilities
- Proficient in electronic medical record (EMR) systems
