This position is based in Melbourne, FL and requires daily travel within Brevard County.

Schedule:

Monday-Friday, 8:00 AM - 5:00 PM.

Salary Range:

$85, annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

Responsibilities

* Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.

* Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.

* Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.

* Functions as a liaison between disciplines and facilities during the admission process.

* Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.

* Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.

* Performs on-call responsibilities and on-call services to patients/families as assigned.

* Performs other duties as assigned.

Qualifications

* Current RN license, specific to the state(s) you are assigned to work.

* 2+ years of clinical RN experience in acute healthcare setting(s)

* Current CPR certification.

* Valid driver's license, reliable transportation, and liability insurance.
