Admissions Nurse

The Admissions Nurse is responsible for coordinating and managing the clinical admissions process to ensure appropriate placement, timely intake, and regulatory compliance.

Key Responsibilities:

• Review and clinically screen referrals for SNF appropriateness

• Coordinate admissions with hospitals, case managers, and internal teams

• Complete and verify admission paperwork, assessments, and authorizations

• Ensure compliance with CMS, Medicare, Medicaid, and facility policies

• Conduct admission assessments and support smooth resident transitions

• Communicate clearly with residents, families, and interdisciplinary teams

Qualifications:

• RN License in the state of Florida

• SNF or post‑acute admissions experience preferred

• Knowledge of Medicare criteria, PDPM, and insurance authorizations

• Strong clinical judgment, organization, and communication skills
