Registered Nurse

Overview

Team Select Home Care is looking for a Registered Nurse (RN) to provide private duty nursing care to medically complex children and adults in the comfort of their homes. The role offers flexible hours and weekly pay options, and focuses on compassionate, patient‑centered care.

• Provide personalized nursing care for pediatric and/or adult patients with complex needs.
• Coordinate care plans with physicians, therapists, and team members.
• Monitor patient progress, update care plans, and document visits in EMR.
• Educate and support families using in‑home care techniques.
• Collaborate with clinical supervisor, participate in case conferences, and act as a trusted team member.
• Perform initial evaluation visits, regular reassessments, and meet state/federal documentation requirements (OASIS, PAR).
• Advocate for patient welfare and coordinate care among providers.
• Administer medication per agency policy and physician orders.
• Maintain medication reconciliation, assess and triage after‑hours calls, and report incidents.
• Participate in quality improvement activities and maintain confidentiality.
• Take on‑call duty nights, weekends, and holidays as assigned.

• Active RN license in the state(s) of practice.
• CPR certification.
• Clinical experience preferred; new graduates welcomed.
• Excellent communication, critical‑thinking, and compassionate presence in a home environment.
• Ability to perform required physical tasks and travel as required.
• Computer skills, competency in EMR, and ability to meet all health requirements for clearances and continuing education.

Pay Range: $23 – $30 per hour, with weekly pay options.
