Must have valid LPN license.Job Summary
Responsible for carrying out the instructions of the Director of Wellness. Supervise and monitor nursing staff to obtain highest quality care by performing routine nursing duties. Accountable to Director of Wellness/Executive Director for all nursing department activities and maintaining the corporate philosophy of quality care.
Duties and Responsibilities- Supervise HHAs, CNAs and facility staff.
- Make daily rounds of all Assisted Living residents and retirement residents if needed.
- Observe and report symptoms and conditions of patients to Director of Wellness (DOW)/Delegating RN/Physician.
- Administer medications and treatments per signed doctor’s orders accurately and safely, observe effectiveness and report to physician if needed.
- Inform DOW of when supplies and equipment are needed.
- Order medication from pharmacy when needed.
- Supervise the Home Health Aide to ensure all nursing care is given properly, safely, and with highest quality.
- Administer emergency first aid.
- Ensure daily assignments for patient care and incidental duties, as outlined by Director of Wellness, are carried out. Nurse/HHA documentation includes V/S, weights, nurse’s notes, medication, kardex, etc.
- Maintain cleanliness and neatness of nurse’s station, med room and cart.
- Assist in orienting new staff.
- Complete admission and discharge evaluations as necessary.
- Check equipment for cleanliness and condition.
- Provide guidance for nursing assistants.
- Promote and maintain good interpersonal relationships with residents, visitors and personnel.
- Perform related work as required per assignment. Understand and execute duties for both assisted living and independent residents throughout the facility.
The above duties include many tasks for LPN. At times it may be necessary to perform other duties as requested by the Director of Wellness. Refusal to perform any task as directed by leadership will be grounds for disciplinary action.

- Graduate of an accredited professional nursing program, currently licensed to practice per state.
- Work experience in the geriatric field.
- Ability to communicate well and in writing.
- Ability to deal with visitors, staff and residents in a courteous manner.
- Ability to lift, push, pull up to 60 lbs.
- Strong organizational skills.
- Ability to work under pressure at times.
- Must maintain good grooming practices, personal hygiene, and be punctual and regular in attendance.
- Must have warm, friendly, congenial personality. Present a professional appearance and embody an empathetic attitude toward seniors.
- Ability to assess a resident’s condition and intervene when needed.
- Must be CPR certified, or obtain certification within 90 days of hire and annually thereafter.
- Must have a valid LPN license.
- May have a picture taken and image used in social media or community advertising.
- May be video recorded from devices installed by families in residents’ apartments.
