What You'll Do:
- Provide skilled nursing care under the direction of an RN
- Administer medications and treatments based on individualized plans
- Educate and support families in-home care techniques
- Observe and report changes in patient condition
- Document visits using our easy-to-use EMR system
- Be a trusted part of a long-term care team focused on safety, comfort, and continuity
What You Bring:
- Active LPN license
- New graduates welcome! We offer a structured training and mentorship program
- CPR certification
- Excellent communication and critical thinking skills
- A passion for working with pediatric and/or adult patients in a home setting
