We are looking for an LPN for our Reading office! We are open to an RN as well.

Hours:

40 hours weekly, Monday-Friday.

It includes performing duties such as operating as part of the health care team under direct supervision of a registered nurse, participating in the implementation and evaluation of patient care, and overseeing the design, implementation, or delivery of processes, programs, and policies using specialized knowledge acquired through advanced education.

Overview

The LPN Outpatient works under the direction of the registered nurse and utilizes the nursing process in the planning, implementing, and evaluation of nursing care to the patient. The role collaborates with other members of the team to provide health information, collect data on the health status of patients, and assure care that reflects the organization's core values. The Licensed Practical Nurse is knowledgeable about and supportive of the primary nursing model.

Job Description

Minimum Qualifications
  • Massachusetts licensed Practical Nurse (LPN).
  • Experience as required by unit needs.
  • Basic Life Support Certification.
Preferred Qualifications
  • Graduation from an accredited practical nursing program.
Duties and Responsibilities
  • Implement the medical/nursing plan of care, nursing interventions, medication administration, and procedures under the direction of the Registered Nurse.
  • Provide direct patient care within the primary nursing model to ensure continuity and safety.
  • Perform technical aspects of patient care (medications, treatments, etc.) safely and in accordance with policies and the Nurse Practice Act.
  • Assess and reassess pain, utilize appropriate pain management techniques, and document pain management.
  • Communicate all changes in the patient's condition immediately to the Registered Nurse.
  • Base all nursing interventions on pathophysiological and psychosocial rationale, delivering care in a fiscally responsible manner.
  • Establish a positive relationship with the patient and family, introduce oneself as the patient's nurse, provide teaching and counseling, and document teaching appropriately.
  • Review/educate the discharged patient and family to ADLs, medications, treatments, and provide discharge instructions.
  • Document all nursing interventions and the patient's response in the medical record.
  • Delegate responsibilities to others based on their roles and capabilities.
  • Participate in performance improvement activities as requested.
  • Maintain professional practice, remain calm and professional, and keep current with evolving practices through lifelong learning.
  • Maintain collaborative relationships with peers and colleagues to create a positive work environment.
  • Report pertinent observations and reactions to the appropriate person (e.g., physicians, clinical manager, charge nurse) accurately and concisely.
Physical Requirements
  • Prolonged, extensive, or considerable standing/walking.
  • Lifts, positions, pushes, and/or transfers patients and equipment.
  • Reaches, stoops, bends, kneels, and crouches.
  • Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise, and possible exposure to radiation, lasers, electric shock, etc.
  • Regular exposure to risk of blood borne diseases and other transmissible infections.
  • Handles emergency and crisis situations.
  • Works irregular hours.
  • May come into contact with hazardous materials.
Skills & Abilities
  • Effectively communicate with patients, families, physicians, and the healthcare team.
  • Apply the nursing process to implement a nursing plan of care.
  • Provide care throughout the lifespan, considering aging processes, human development stages, and cultural patterns.
  • Provide age-appropriate communication and interpret age-specific data responses.
  • Demonstrate knowledge of pharmacology/medications in administering drugs and monitoring patient responses.

Pay Range: $30.00 – $33.59
