Tufts Medicine’s Melrose Wakefield Hospital is seeking a Social Worker - Case Manager for a full time day shift. You will work within an interdisciplinary team to assess, advocate, and coordinate care from admission to discharge, ensuring alignment with case management standards and patient-centered goals.
Minimum qualifications include a Master’s degree in Social Work, LICSW or LCSW licensure, and at least three years of social work experience, plus current BLS.
