Social Worker - Case Manager​/Day Shift

We are seeking a dedicated Social Worker - Case Manager to join an interdisciplinary team, providing comprehensive assessments, care planning, and advocacy to ensure safe and effective patient discharges.

• Assessing discharge planning needs within 24 hours of admission and coordinating post-acute services.
• Participating in multidisciplinary meetings to identify and resolve discharge barriers.
• Educating patients and families about discharge plans, addressing Social Determinants of Health, and managing payor matters.
• Ensuring timely authorization for post-acute care and maintaining accurate documentation.
• Supporting ethical decision-making, advance directives, and surrogate decision-makers.
• Delegating tasks to unlicensed team members and mentoring colleagues.
• Conducting psychosocial assessments, advocating for high-risk populations, and coordinating community resources.
• Identifying risk factors for crisis intervention and implementing strategies to prevent readmissions.
• Managing complex cases involving abuse, neglect, domestic violence, and other at-risk situations.

• Master’s degree in Social Work (MSW).
• LICSW or LCSW licensure.
• At least three years of social work experience, preferably in acute care.
• Current BLS certification.
• Preferred:
  Case Management certification and hospital setting experience.

Strong assessment, communication, advocacy, and interdisciplinary collaboration skills, with a focus on patient-centered care and community resource coordination.
