Job Details

The Outreach Program Coordinator is responsible for the seamless logistical execution of Catholic Health’s community outreach initiatives. This role ensures that our mobile units and health screening programs reach the most vulnerable populations by managing site scheduling, volunteer coordination, and data integrity. The Coordinator serves as the central point of contact for sites, RNs, and drivers to ensure every outreach event is a success.

Key Responsibilities

Program Logistics & Scheduling

  • Site Management:
    Identify and book new outreach sites utilizing the Vizient Vulnerability Index to ensure data-driven community placement.
  • Calendar Oversight:
    Confirm site visits one month in advance and perform final re-confirmations the week prior to the event.
  • Internal Communications:
    Distribute weekly and monthly bus schedules to staff, RNs, and marketing platforms like Wrike for website updates.
  • Cancellations & Pivot Planning:
    Rapidly notify all internal teams (Office/RNs/Drivers) and update digital schedules in the event of site cancellations.

Administrative & Data Excellence

  • Clinical Records:
    Perform accurate entry of patient records into Epic and maintain community benefit statistics in CBISA.
  • Annual Screenings:
    Manage the BP screening workflow, including annual confirmation letters and statistical workbook maintenance.
  • Office Operations:
    Oversee budget management, handle purchasing/payments, and maintain inventory of all office and outreach supplies.

Mobile Unit & Outreach Support

  • Compliance & Safety:
    Maintain outreach bus/van records, including vehicle registrations and securing Certificates of Insurance (COI) for site visits.
  • Fleet Maintenance:
    Coordinate with bus drivers to schedule necessary vehicle repairs and routine maintenance.
  • Community Promotion:
    Create and distribute bi-monthly outreach flyers and monthly promotional materials to sites to maximize patient turnout.
  • Volunteer Engagement:
    Assist in securing and coordinating Healthy Sundays volunteers for community events.

Qualifications:

  • Technical Proficiency:
    Ability to learn and navigate Epic (Clinical Records), CBISA (Community Benefit), and Wrike (Project Management).
  • Logistical Coordination:
    Strong ability to manage complex schedules and multi-task in a fast-paced environment.
  • Communication:
    Professional phone and email etiquette for interacting with community partners and internal healthcare leads.
  • Organization:
    Acute attention to detail for maintaining vehicle compliance and financial records.
Salary Range

USD $24.00 - $28.00 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
