Hospice Aide, Certified Home Health Aide

Northwell Health -  - Responsibilities:
Provide patient care in accordance with plan of care established by Hospice Care Registered Nurse and other interdisciplinary group members.;
Assist patients with activities of daily living, including bathing, grooming, shaving, oral hygiene, skin care, perineal care, foot and heel care, turning and positioning, ROM exercises, ambulation, and changing bed linens.;
Document care provided and maintain accurate ADL records; ensure documentation is complete and legible.;
Perform light housekeeping tasks and manage supplies to keep the patient's area clean and stocked.;
Report changes in patient condition or needs to the Hospice RN or Team Manager; participate in on-call rotations and attend in-service trainings.
