Job Details- Compensation: LPN $35 / hour, RN $45 / hour

- Active LPN or RN license issued by NYS
- Minimum of 1 year experience in a home‑care setting
- Current BLS certification
- Empathy, reliability, and a commitment to high‑quality care
- Perform physical and health assessments before making critical decisions
- Provide health promotion, counseling, and education
- Administer medications and other personalized interventions
- Coordinate care in collaboration with a wide array of health‑care professionals
- Document nursing notes regarding patient care daily
- Must be able to lift 50 lbs and stand/work for extended periods
- Additional responsibilities as required
