What You’ll Do

As a Social Worker, you will advocate for residents’ well‑being, conduct psychosocial assessments, develop individualized care plans and provide counseling.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments.
  • Collaborate with residents, families and interdisciplinary team to develop individualized care plans.
  • Provide counseling and emotional support on coping strategies, transitions and decision‑making.
  • Advocate for residents’ rights and preferences.
  • Connect residents and families with community resources and support services.
  • Offer crisis intervention during challenging situations.

Rate of Pay

Starting from USD $32.00/Hr.

What You’ll Bring
  • Master’s degree in social work from an accredited program.
  • Current licensure or certification in the state of employment.
  • Previous experience in long‑term care, geriatrics or related field.
  • Strong assessment, counseling and crisis intervention skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Empathy, compassion, and commitment to person‑centered care.
