the Facility
Cordova Wellness & Rehab Center is a 242-bed skilled nursing facility offering:
- Short-term rehabilitation
- Long-term care
- Outpatient therapy services
Located in the East Memphis/Cordova area, this in-house rehab team is known for strong leadership, a supportive culture, and a commitment to excellent clinical outcomes.
Responsibilities- Provide skilled occupational therapy treatments in accordance with the supervising Occupational Therapist’s established plan of care.
- Assist the OT with evaluations, tests, and complex therapeutic procedures.
- Observe, document, and report resident conditions, responses, and progress.
- Maintain accurate daily treatment documentation and progress notes.
- Assist in consultations with families, physicians, and interdisciplinary team members.
- Support care, cleanliness, and organization of the OT department and equipment.
- Participate in in-service training, staff meetings, and continuing education.
- Contribute to care conferences and quality improvement initiatives.
- Follow all Ahava Therapy Services policies, regulatory requirements, and professional standards.
- Understand payer guidelines and allocate therapy resources based on patient needs and discharge goals.
- Demonstrate knowledge of OBRA regulations, HIPAA, LCD policies, and third-party reimbursement.
- Maintain expected productivity levels while demonstrating strong teamwork, communication, and clinical judgment.
- Graduate of an accredited Occupational Therapy Assistant program.
- Active Tennessee OTA license (or eligibility).
- Current BLS/CPR certification (or obtained during orientation).
- Strong written, verbal, and observational skills.
- Ability to work collaboratively within an interdisciplinary team.
