TRA RN Case Manager - Discharge Planning

* This position is onsite every day at St Francis Hospital - Park. 8 am to 4:30*

This RN Case Manager / Discharge Planning contract position requires 2 - 3 years Acute Care Discharge Planning experience. You must have STRONG two years of Discharge Planning / Care Coordination in an Acute Care Hospital Environment to be considered. Experience with an Insurance company does not qualify and will not be considered.

• The RN Case Manager is responsible to facilitate care along a continuum through effective resource coordination
• Transition Management promoting appropriate length of stay, readmission prevention and patient satisfaction
• Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care
• Compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy
• Education provided to physicians, patients, families and caregivers

• Must have BLS
• Must be actively licensed as an RN in Tennessee.
• Must have two - three or more years of working with in Case Management / Discharge Planning for an Acute Care hospital.
• Must have worked with a patient case load of, at least, 40.
