Job Description

Position Summary

This position is funded by a grant for a duration of time as defined by the grantor.

Under managerial supervision, this position engages individuals with substance use disorders in Shelby County’s high-impact drug areas byfacilitatingaccess to treatment services, providing harm-reduction outreach, and collaborating with community partners to improve health outcomes.

Pay Grade: 51

Salary Commensurate with Experience and Education

Minimum Qualifications

  • Three (3) years of experience in counseling, social work, or case management within apublic health environment or a social service agency or related field;

    AND
  • Bachelor’s degree from an accredited college or universityinsocial work, criminal justice, psychology, sociology, or a closely related field; OR
  • An equivalent combination of related education and/or experience.
  • Individuals with lived experience in substance use or substance disorder preferred.
  • PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED.

Duties and Responsibilities

  • Screens and engages participants for navigation services, coordinating with hospitals and service providers to support substance use intervention and recovery efforts.
  • Refers individuals to substance use disorder treatment, medication-assisted treatment (MAT), harm reduction services, and Syringe Service Programs (SSP), ensuringtimelyaccess to care.
  • Provides harm reduction education, recovery resources, and navigation support to individualsimpactedby substance use.
  • Conducts outreach andparticipatesin recovery-focused initiatives and events, including START (Strategic Team Accessing Recovery Treatment) and other community-based engagement efforts.
  • Collaborates with other Peer Navigator Coordinators and community partners toidentifytrends in substance use, demographics, and service needs across Shelby County.
  • Collects,maintains, and analyzes participant and program data; prepares reports, deliverables, and tracking tools for management review and program evaluation
  • Builds community awareness through education, presentations, media support, and informational materials for residents, organizations, and stakeholders.
  • Participates in task force meetings, opioid response meetings, and professional development activities to remain current on best practices and emerging trends in substance use services.
  • Performs other related duties as requiredor directed.

KSAs

  • Ability to effectively engage, screen, and build rapport with individuals experiencing substance use challenges using trauma-informed approaches.
  • Knowledge of substance use disorders, harm-reduction principles, recovery services, and medication-assisted treatment (MAT).
  • Skilled in navigating individuals to appropriate treatment, community resources, and social support services.
  • Ability to collaborate with healthcare providers, community partners, and multidisciplinary teams to support coordinated care.
  • Ability to collect, document, and report program data accurately whilemaintainingconfidentiality and compliance standards.
  • Proficientknowledgein Microsoft Officeor comparable county systems.

Special Requirements

  • May berequiredto work extended hours, including weekends,and available to work on call during an event.
  • Work will require travel to locations outside of the officeand community events.
  • Must obtain Community Health Worker certification within one (1) year of employment.
  • Mustmaintainall required licenses,certifications, and training credentials throughout the duration of employment.
