Since 9, Lakeside has been the premier provider of specialized behavioral health and addictive disease services in the Memphis and Mid‑South region. The
-bed facility is one of the largest free‑standing psychiatric hospitals in the United States. We are located on a serene
-acre campus in northeast Shelby County, serving as the setting for the treatment for children, adolescents, young adults, adults and seniors. Lakeside’s comprehensive programs treat disorders such as bipolar disorder, psychological trauma, depression and anxiety, mental illness and addictive disease disorders such as alcoholism, drug addiction and substance abuse.
The Discharge Planner provides discharge planning and case management services, crisis intervention and liaison services with referral sources and other departments within the hospital in a manner consistent with the philosophy and objectives of Lakeside Behavioral Health System.

JOB SPECIFICATIONS:
To perform this job successfully, an individual must be able to demonstrate competency in the criteria listed in this job description. The specifications listed below are representative of the knowledge, skill, and ability to meet the minimum requirements for this position. Staff must demonstrate flexibility for reassignment to units providing care to varied populations including, adults and dually diagnosed patients in an inpatient and partial hospital setting.
Education:
Bachelor’s degree in Social Work, Clinical/Counseling Psychology or Social Science related area required, licensure preferred.
Experience:
Prior experience working with a psychiatric mental health population, preferably in an inpatient setting.
Licensure/Certification: not required
Knowledge:
Prefer knowledge of case management, discharge planning, family therapy theory and practice.
