The Medical Social Worker (LMSW) provides medical social work services, psychosocial assessment, and counseling for patients and their families in close conjunction with the physician and other health care team members.
Responsibilities- Conduct comprehensive social work assessments and document findings within 24 business hours of referral.
- Identify and prioritize social work interventions, with special consideration given to discharge planning and the patient’s right of self-determination.
- Provide individualized counseling, support groups, crisis intervention, and referrals for psychotherapy or other mental health services.
- Coordinate discharge planning, facilitate communication with community service providers, and support patients in accessing equipment and outpatient services such as DME, hearing/optical appliances, nursing home, home care, and dialysis.
- Initiate and manage referrals to agencies and collaborate with multidisciplinary team members to ensure effective care transitions.
- Maintain accurate and timely documentation in the medical records system, generate statistics, tables, and charts, and prepare quarterly Quality Council reports.
- Assist in the development of policies, training, and performance improvement initiatives related to social work services.
- Adhere to HIPAA, hospital compliance, and other regulatory requirements, and maintain patient confidentiality.
- Participate in staff training, orientation of medical staff, residents, and students, and stay current with best practices through continuing education.
- Master’s Degree in Social Work from an accredited college or university.
- Licensed Medical Social Worker (LMSW) with state licensure in Tennessee, required upon hire.
- Qualified by education, training, or experience to work with adolescent, adult, and/or geriatric patient populations.
- Minimum 1 year of social work experience preferred.
- Standing, walking, lifting, carrying, pushing, pulling, climbing, balancing, stooping, kneeling, crouching, crawling, reaching, handling, grasping, feeling, talking, hearing, repetitive motions, and eye/hand/foot coordination are all performed frequently or constantly as required.
