Recovery Support Specialist

The Recovery Support Specialist ensures that clients who meet the eligibility criteria for mental health treatment and care coordination services under the Shelby County-funded Jericho jail diversion program are linked to and receive those services as needed. Responsibilities include completing assessments prior to and following jail release, developing and coordinating individualized treatment and Community Linkage Plans, providing intensive care coordination support during the critical post-release period, and providing care coordination services (billable when possible) for Jericho clients and Alliance outpatient clients.

This position integrates recovery support, jail diversion case management, and billable care coordination responsibilities within the Jericho program.

Supervisory Responsibility:
None.

Education Requirement:
Bachelor's degree in human services, psychology, social work, counseling, or a related field required.

Licensure/Certification Requirement:
Must meet Tenn Care credentialing standards.

Experience Requirement:
Experience working with adults with serious and persistent mental illness, co-occurring disorders, and involvement in the criminal justice system preferred. Must demonstrate strong assessment, care coordination, and case management skills, along with the ability to work effectively with consumers during crisis and post-incarceration transition.

Key Job Responsibilities

Assessment and Intake

• Facilitate functional assessment of mental health treatment needs, including determination of options for securing and maintaining resources, and coordinate acceptance/approval of clients into the Jericho program.
• Provide functional assessment services prior to and following jail release, including on-site assessments at the correctional center when possible.
• Complete Community Linkage Plans for clients on the Jericho caseload.

Care Coordination and Resource Linkage

• Provide face-to-face and community-based care coordination services- billable when possible- in accordance with Tenn Care, TDMHSAS, and agency requirements, to support consumer linkage to treatment and community resources. This includes backup care coordination services for outpatient clinics when needed and directed.
• Facilitate referrals to appropriate mental health resources and community assistance programs, assisting consumers in achieving maximum benefit from those services.
• Ensure, as directed by the Grant Manager and Clinical Manager, appropriate utilization of Jericho funds designated for critical unmet needs, such as safe housing, continuity of medications during the resource acquisition process, and other needs determined essential to successful community linkage.
• Provide transportation for consumers when clinically appropriate and in accordance with agency transportation requirements.
• Participate in the Jericho Roundtable meetings with Shelby County and facilitate ongoing communication and feedback between program partners, including development and presentation of Community Linkage Plans.

Compliance, Collaboration, and Professional Expectations

• Ensure that services are provided in accordance with regulations established by the Center's funding and regulatory sources, including Shelby County, SAMHSA, the State of Tennessee, and Tenn Care.
• Comply with Alliance Healthcare Services' policies and procedures manual.
• Complete all required trainings, meetings, documentation, and compliance requirements within established time frames.
• Perform other duties as assigned by supervisors.
