The Heart Failure Program Coordinator provides professional, clinical nursing, and technical knowledge, skills, and expertise for the development and continuous improvement of comprehensive cardiac and/or heart failure patient care. Researches, develops, and applies evidence based practice in clinical care, patient and staff education, and the development and adherence of policies, procedures and treatment protocols, and overall plan of care. Provides data management oversight, data analysis, and reporting to identify patterns of care, benchmarked best practices, and opportunities for improvement, as well as concurrent reviews of the heart failure patients.

A Brief Overview
The Heart Failure Program Coordinator provides professional, clinical nursing, and technical knowledge, skills, and expertise for the development and continuous improvement of comprehensive cardiac and/or heart failure patient care. What You Will Do
- Assessment:
Provides clinical expertise to evaluate cardiac patients, heart failure protocol/processes, and associated data collection systems to support effective patient care delivery. - Problem/Diagnosis:
Coordinates with the healthcare staff to identify and diagnose patient care problems, implement approaches to these problems, and evaluate the effectiveness of the interventions. - Coordinates the development and implementation of an educational program designed to convey specialized knowledge and skills to the healthcare staff, patients, families, and the community.
- Coordinates Cardiac Care Program goals using a multidisciplinary approach.
- Actively acts as a liaison for Emergency Medical Services to include collaboration, communication, education, process improvement and case reviews.
- Actively identifies, plans, and participates in research related to cardiac patients.
- Actively reviews day to day heart failure patients and communicates needs with the involved staff.

- Bachelor's Degree Nursing

- Clinical nursing 5-7 years
Licenses and Certifications Requirements
- Required - BASIC LIFE SUPPORT
- American Heart Association - Required
