About the RoleMatter Health is seeking an Advanced Practice Provider/Nurse Practitioner to support older adults across Memphis, TN. This role may be full‑time or part‑time depending on market needs.
At Matter Health, our APPs are more than providers — they are Panel Owners. You will manage a dedicated panel of older adults, serving as their longitudinal primary care provider while building trusted relationships directly within the communities you serve.
This is a fully community‑based role with no traditional clinic home base. APPs operate with a high level of clinical autonomy while partnering closely with an interdisciplinary care team and dedicated Medical Director for collaboration and support with complex cases.
What You’ll Do- Provide longitudinal primary and preventive care for older adult patient populations
- Manage and grow a dedicated patient panel within assigned communities
- Deliver patient‑centered, relationship‑driven care directly in community settings
- Collaborate closely with interdisciplinary clinical and operational teams
- Support preventative care, chronic disease management, and care coordination initiatives
- Utilize telehealth and in‑person care delivery models to improve healthcare accessibility
- Maintain accurate and timely EHR documentation and coding specificity
- Active, unrestricted Nurse Practitioner or Physician Associate license
- NP (AANPCB or ANCC — family, adult, geriatric, or adult‑gerontology)
- PA (NCCPA)
- Active DEA license and controlled substance registration where applicable
- 3+ years of clinical experience as an APP, preferably with aging or medically complex populations
- Valid driver’s license and reliable transportation (car stipend provided)
- Experience with EHR documentation (athenahealth experience preferred)
- Commitment to equitable, patient‑centered care
- Background in family medicine, internal medicine, adult medicine, or geriatrics
- Experience in value‑based care environments
- Experience providing care in community‑based, in‑home, or non‑traditional healthcare settings
- Familiarity with MEAT documentation and chronic condition coding specificity
