Matter Health is seeking an Advanced Practice Provider/Nurse Practitioner to support older adults across Memphis, TN. This role may be full‑time or part‑time depending on market needs.

At Matter Health, our APPs are more than providers — they are Panel Owners. You will manage a dedicated panel of older adults, serving as their longitudinal primary care provider while building trusted relationships directly within the communities you serve.

This is a fully community‑based role with no traditional clinic home base. APPs operate with a high level of clinical autonomy while partnering closely with an interdisciplinary care team and dedicated Medical Director for collaboration and support with complex cases.

• Provide longitudinal primary and preventive care for older adult patient populations
• Manage and grow a dedicated patient panel within assigned communities
• Deliver patient‑centered, relationship‑driven care directly in community settings
• Collaborate closely with interdisciplinary clinical and operational teams
• Support preventative care, chronic disease management, and care coordination initiatives
• Utilize telehealth and in‑person care delivery models to improve healthcare accessibility
• Maintain accurate and timely EHR documentation and coding specificity

• Active, unrestricted Nurse Practitioner or Physician Associate license
• NP (AANPCB or ANCC — family, adult, geriatric, or adult‑gerontology)
• PA (NCCPA)
• Active DEA license and controlled substance registration where applicable
• 3+ years of clinical experience as an APP, preferably with aging or medically complex populations
• Valid driver’s license and reliable transportation (car stipend provided)
• Experience with EHR documentation (athenahealth experience preferred)
• Commitment to equitable, patient‑centered care

• Background in family medicine, internal medicine, adult medicine, or geriatrics
• Experience in value‑based care environments
• Experience providing care in community‑based, in‑home, or non‑traditional healthcare settings
• Familiarity with MEAT documentation and chronic condition coding specificity
