- Help manage order tracking, IDG preparation, and other tasks associated with the management of patient care.
- Monitor pending referrals daily and assign registered nurse or advanced practice nurse for all start of care visits.
- Monitor productivity and distribution of visits and communicate any needed changes with the clinical leader, as appropriate.
- Ensure that the patient visit schedule supports continuity of care and excellence in patient care management by scheduling most nursing visits to the patient s designated RN Case Manager, Social Worker.
- Participate in patient care team meetings to assist with facilitating timely care via the scheduling process for patients with symptom crises, transitioning to end of life, or other immediate physical, psychosocial, or spiritual care needs.
- Assist with coordination, scheduling, and tracking of all visits to ensure compliance with hospice policies and state and federal regulatory requirements.
- Required Qualifications
- Current LPN/LVN license in state of practice
- Current driver s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
- Preferred Qualifications
- Current CPR certification or ability to complete within 90 days of hire
Pay is based on several factors including local labor markets, education, work experience, certifications, etc. In addition to salary, benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (eligibility requirements apply). The hourly pay range for this role is $27.25 to $40.90 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
