The Licensed Practical Nurse (LPN) in a Skilled Nursing Facility is responsible for delivering high‑quality nursing care to residents under the supervision of a Registered Nurse (RN) or Nurse Manager. The LPN works collaboratively with the healthcare team to ensure the well‑being and comfort of residents.

• Conduct initial and ongoing assessments of residents’ health status.
• Monitor vital signs, administer medications, and observe any changes in condition.
• Notify the RN or healthcare provider of any significant changes or concerns.
• Administer medications as prescribed by the healthcare provider.
• Ensure accurate dosage, proper route, and timely administration.
• Monitor and record residents’ responses to medications.
• Provide basic wound care and other treatments as ordered by the healthcare provider.
• Monitor the healing progress of wounds and report any complications.
• Work closely with Registered Nurses, Nurse Managers, Certified Nursing Assistants (CNAs), and other healthcare professionals to coordinate care.
• Participate in care planning meetings and contribute to the development of resident care plans.

• Must be a Licensed Practical Nurse (LPN) in TN or compact.
