What You'll Do

  • As a Home Health RN, you'll provide skilled, compassionate care while working independently and collaborating closely with physicians and an interdisciplinary care team.

Your responsibilities will include:

  • Completing comprehensive patient assessments, admissions, and ongoing care planning
  • Coordinating care with physicians and the interdisciplinary healthcare team
  • Delivering skilled nursing services, including therapeutic, preventive, and rehabilitative care
  • Monitoring patient progress and updating individualized plans of care
  • Educating and supporting patients and their families throughout their healthcare journey
  • Maintaining accurate, timely clinical documentation while ensuring compliance with regulatory standards
  • Identifying changes in patient condition and communicating updates to the appropriate members of the care team
  • Promoting patient independence, safety, and overall quality of life in the home setting

What You'll Need

  • Current Registered Nurse (RN) license in the state of Tennessee
  • Graduate of an accredited nursing program
  • Minimum of one (1) year of clinical nursing experience (ICU, Medical-Surgical, or Rehabilitation preferred)
  • Home Health experience preferred, but not required
  • Current CPR certification
  • Valid driver's license, reliable transportation, and current automobile insurance
