What You'll Do
- As a Home Health RN, you'll provide skilled, compassionate care while working independently and collaborating closely with physicians and an interdisciplinary care team.
Your responsibilities will include:
- Completing comprehensive patient assessments, admissions, and ongoing care planning
- Coordinating care with physicians and the interdisciplinary healthcare team
- Delivering skilled nursing services, including therapeutic, preventive, and rehabilitative care
- Monitoring patient progress and updating individualized plans of care
- Educating and supporting patients and their families throughout their healthcare journey
- Maintaining accurate, timely clinical documentation while ensuring compliance with regulatory standards
- Identifying changes in patient condition and communicating updates to the appropriate members of the care team
- Promoting patient independence, safety, and overall quality of life in the home setting
What You'll Need
- Current Registered Nurse (RN) license in the state of Tennessee
- Graduate of an accredited nursing program
- Minimum of one (1) year of clinical nursing experience (ICU, Medical-Surgical, or Rehabilitation preferred)
- Home Health experience preferred, but not required
- Current CPR certification
- Valid driver's license, reliable transportation, and current automobile insurance
