Registered Nurse – Home Health

Home Health Registered Nurse

Seeking an experienced registered nurse for a Home Health position requiring travel and on-call availability. The ideal candidate will provide in-home patient care while adhering to strict compliance policies and maintaining clinical excellence.

Responsibilities:

• Assess patients’ health status and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Educate patients and families on health management and disease prevention.
• Document patient interactions and clinical findings accurately in electronic health records.
• Coordinate with healthcare teams to ensure seamless patient care delivery.
• Respond to on-call requests, providing urgent patient assessments as needed.

Qualifications:

• Must have a minimum of 2 years of recent RN experience, with 3+ years preferred.
• Hold an active RN license in the state or a valid compact license per NLC guidelines.
• Have completed at least one prior travel assignment.
• Demonstrate proficiency with EPIC electronic health record system.
• Be willing to work Monday through Friday, 8:00 am to 5:00 pm, with call shifts.
• Adhere to travel compliance requirements, including a minimum of 100 miles from the facility if local.
