What Youll Do As a Home Health LPN youll provide skilled nursing care directly in patients homes while working closely with the Registered Nurse and interdisciplinary care team Responsibilities include Providing direct patient care according to the established plan of care Completing skilled nursing visits in patients homes

Administering treatments and medications as ordered

Monitoring patients and reporting changes in condition to the RNEducating patients and families on medications treatments and self care Completing accurate and timely clinical documentation

Following infection control and safety procedures

Collaborating with the RN physicians therapists and other members of the care team Supporting patients in managing chronic conditions and maintaining independence at home Qualifications Current LPNLVN license in Tennessee Graduate of an accredited practical or vocational nursing program

Current CPR certification

Reliable transportation

Valid drivers license and current automobile insurance3 years of nursing experience preferred
