Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse team in Memphis, TN. You will provide skilled 1:1 pediatric home care for high-acuity patients, including trach, vent, and G-tube management.
As a Transition Nurse, you’ll ensure safe hospital-to-home discharges, prevent readmissions, and cover interim care until permanent staff are in place.
