We are seeking a Registered Nurse Case Manager to develop and implement person-centered care plans for individuals with intellectual and developmental disabilities.
Key responsibilities- conducting client assessments
- creating and reviewing care plans
- coordinating home care services
- ensuring compliance with medical standards and policies
- training and supervising aides
- maintaining effective communication with clients, families, and care providers
- a Bachelor's Degree in Nursing
- a valid nursing license
- at least two years of experience
- strong clinical knowledge
- excellent communication
- the ability to perform supervisory visits
- travel as needed
- physical ability to perform heavy work
