Hiring Salary: $20.65/Hourly

This is a grant-funded position funded until June 30, 2027. The funding may be extended pending funding availability.

Under the direct supervision of the Program Manager for the Shelby County Relative Caregiver Program, the Family Advocate Counselor performs intakes, referrals, home visits, case management, maintaining client records (files), planning, organizing events, and facilitating the Boys/Girls mentoring group both in person and via zoom monthly. This position exists to support and assist caregivers in determining specific needs and obtaining needed support with projects resources through the network and/or other providers.

This position will allow hands-on contact with individuals and families through home and office visits.

Responsibilities
  • Identifies participant's eligibility through intakes daily via phone, walk-ins, and referrals from other agencies.
  • Conducts home visits to assess family eligibility and environment-include monthly and/or quarterly contacts.
  • Develops an individualized Action plan – Action plan is developed in collaboration with the caregiver in order for the family to receive appropriate services.
  • Determines specific needs of families through assessments and interviews with the family.
  • Completes family redeterminations, case graduations, and case closures.
  • Provides monthly or quarterly face-to-face meetings or phone contact with program participants.
  • Maintains accurate case notes on clients from initial contact up to and ending with case closure; submits accurate monthly reports denoting the number of client contacts and activities, successes, and challenges.
  • Collaborates with outreach agencies to secure additional resources to gain greater family stability.
  • Attends workshops pertaining to family preservation; links families to supports and services in the community.
  • Performs other related duties as assigned.
  • Qualifications

    Education:

    Bachelor's Degree. (Transcript required)

    Experience:

    Two (2) years of experience working with children and families; OR Master's Degree and one (1) year of experience of experience working with children and families; OR a combination of education and experience to equal 6 years.

    Knowledge, Skills, and Abilities:

    • Knowledge of community and social services resources.
    • Ability to perform home visits. Must be able to work closely with families and program team members in a positive manner.
    • Knowledge of basic computer skills (Microsoft Word, PowerPoint, Access, Excel, etc.).
    • Knowledge of local and state community resources and services.
    • Ability to work with families and children in crisis.
    • Ability to be empathic and a good listener.
    • Ability to facilitate and manage group sessions.
    • Ability to work independently, efficiently and to accomplish both routine and detailed tasks.
    • Ability to be prompt, detail-oriented, courteous, efficient, and professional.
    • Ability to maintain confidentiality and professionalism as it concerns information about staff, families, and program matters.

    License/Certification:
    Valid Driver's License Required (Copy of license/certification required)

    Work Schedule:

    This position frequently works weekends and occasionally work evenings. This position requires travel within Shelby County, with occasional overnight travel.
