A community-based organization is seeking a compassionate and motivated Case Manager to support individuals and families in overcoming barriers to employment and achieving long-term self-sufficiency. This role provides personalized case management, employment readiness support, and connections to community resources while helping participants develop meaningful career and life goals.
This position requires travel between multiple community locations throughout the Memphis area.
Key Responsibilities- Manage an assigned caseload and provide ongoing case management services.
- Conduct assessments to identify employment barriers and participant needs.
- Develop individualized action plans and monitor participant progress.
- Connect participants with workforce development, education, childcare, and community support services.
- Provide job readiness coaching, employment counseling, and job search assistance.
- Coordinate services with internal teams and community partners.
- Monitor compliance with program requirements and maintain accurate case documentation.
- Follow up with participants to support job placement, retention, and long-term success.
- Participate in team meetings and contribute to continuous program improvement.
- Maintain timely, accurate records in accordance with program guidelines.
Required
- High school diploma or equivalent.
- 4–6 years of experience in case management, workforce development, social services, human services, career counseling, or a related field (or an equivalent combination of education and experience).
- Strong organizational and time management skills.
- Excellent verbal and written communication skills.
- Ability to manage multiple priorities in a fast-paced environment.
- Ability to work independently while collaborating with a team.
- Reliable transportation and willingness to travel between program locations.
Preferred
- Experience supporting individuals facing barriers to employment.
- Knowledge of local workforce resources and community organizations.
- Experience developing employment or service plans.
- Strong conflict resolution and problem-solving skills.
- Proficiency with Microsoft Office, including Word and Excel.
- Passionate about helping others achieve employment and personal goals.
- Empathetic, patient, and relationship-focused.
- Organized with strong follow-through and documentation skills.
- Comfortable working with diverse populations and navigating complex situations.
- Self-motivated and committed to making a positive impact in the community.

Pay: $19.00/hour
Schedule: Monday–Friday | 8:00 AM – 5:00 PM
