Position Type: Full-Time
Schedule: Monday–Friday, 9:00 AM–5:00 PM
Compensation: Starting Salary $51,000 Annually (Based on experience)

Position SummaryThe Social Worker is a key member of HEAL 901’s mental health team, providing trauma‑informed mental health support to youth and young adults ages 14–24 who participate in HEAL 901 programs or request services through our mental health office. This role aligns with Cure Violence Global logic models by reducing risk factors, increasing protective factors, and strengthening participant stability through assessment, crisis intervention, referrals, and ongoing case management.
Social Workers serve as clinicians, advocates, and connectors—helping participants navigate trauma, mental health needs, school challenges, employment barriers, and community safety concerns.
Key Responsibilities Mental Health Assessment & Intake- Conduct biopsychosocial assessments for youth ages 14–24
. - Identify trauma exposure, mental health needs, risk factors, and protective factors.
- Develop individualized service plans aligned with goals.
- Provide trauma‑informed crisis response for youth experiencing distress or conflict.
- Create individualized safety plans and coordinate with CURES team members.
- Support de‑escalation during high‑risk situations.
- Connect participants to HEAL 901’s mental health office or external providers.
- Coordinate referrals for therapy, psychiatric evaluation, substance use treatment, and wraparound supports.
- Conduct follow‑ups to ensure service access and continuity.
- Provide ongoing case management and stabilization support.
- Assist participants with school re‑engagement, employment pathways, and resource navigation.
- Conduct home visits, school visits, and community‑based check‑ins as needed.
- Work closely with Outreach Workers and Community Safety Specialists.
- Participate in case reviews, multidisciplinary meetings, and required trainings.
- Engage with schools, clinics, youth-serving agencies, and community partners.
- Maintain accurate, audit‑ready documentation including assessments, service plans, progress notes, referrals, and crisis logs.
- Enter data into approved systems within required timelines.
- Participate in data quality checks and compliance reviews.
- Bachelor’s or master’s degree in social work, Counseling, Psychology, or related field (MSW preferred).
- Experience working with youth ages 14–24 or trauma‑impacted populations.
- Strong crisis intervention, communication, and documentation skills.
- Knowledge of Memphis neighborhoods and youth-serving resources.
- Flexibility to work evenings and weekends based on program needs.
