The Registered Nurse Preceptor is responsible for mentoring, orienting, educating, and training assigned professional and unlicensed client care personnel to help them transition into their roles. They evaluate the clinical competency of employees assigned by the Clinical Supervisor or designee to ensure quality standards of care are met, oversee home health services for clients within the community, and perform comprehensive client care and case management as assigned.
SCHEDULE- Full‑time position, Monday - Friday with on‑call rotation.
- Reports to the Mendota Heights, MN office and provides services in Mendota Heights and surrounding cities.

- Provide direct instruction and guidance on clinical procedures, protocols, and best practices to clinical staff in all client care areas
- Perform initial and periodic assessments of clients to determine care needs, implement interventions, and evaluate effectiveness to achieve positive outcomes
- Collaborate with physicians/non‑physicians and the home health team to develop and update client care plans at least every 60 days or more often as needed
- Coordinate client care with family, caregivers, community resources, and other health care members
- Act as client advocate ensuring safety needs are addressed by assessing the home environment, developing and implementing individualized abuse prevention plans, and reporting suspected vulnerability such as abuse, neglect, or financial exploitation
- Meet documentation standards ensuring accuracy, timeliness, and compliance with payer/billing requirements
- Provide education to clients, families, caregivers, licensed clinicians, and unlicensed personnel
- Provide direct care to clients of varying ages, clinical acuity levels, and diagnoses in accordance with company policies and the Minnesota Nurse Practice Act
- Provide clinical oversight, education, orientation, and training to licensed clinicians, office personnel, and unlicensed personnel
- Perform RN case manager job responsibilities as delegated by the Clinical Supervisor or designee
- Assist in ensuring recruitment, onboarding, evaluation, training, assignment, and discharge of staff are done according to established policies and procedures
- Ensure effective and appropriate financial management of clients’ home health services
- Manage work schedule effectively, adapt to unexpected changes, and participate in the on‑call process
Client care provided in individual client homes and may require transfers and lifting with adaptive equipment. Must be adaptable to a variety of environments and community settings and be able to drive or use public transportation in all types of weather. Heavy: 50–100 pounds occasionally; 25–50 pounds frequently; 10–20 pounds constantly. Light: up to 20 pounds occasionally.
Qualifications- Currently licensed as a Registered Nurse (RN) in the State of Minnesota, license not revoked, suspended, or limited
- Proof of negative TB testing:
Mantoux skin test, single TB blood test, or chest x‑ray - Access to dependable automobile for multiple company business stops a day
- Valid driver’s license and proof of current car insurance when using personal vehicle for company business
- U.S. citizenship or evidence of valid Alien Work Permit
- Disclose any conviction and criminal history related to health services; a candidate convicted of such crimes will not be hired
- Pass initial and ongoing background studies and screenings including those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities
- At least two (2) years of experience in a health care facility or home care field
- Demonstrated critical thinking skills, ability to follow standards and procedures, and make appropriate clinical judgments for safe client care
