Bella Terra is part of the

** Key Responsibilities
*** Implement standards of care for occupational therapy services in accordance with the home health plan of care.
* Provide
** patient, caregiver, and family education and counseling**.
* Communicate effectively with physicians and healthcare team members.
* Evaluate patient’s functional status (muscle function, endurance, coordination, communication skills, self-care ability, etc.).
* Assess home environment for safety and recommend adaptive equipment.
* Develop individualized treatment programs and set functional goals.
* Train patients in daily living activities, prosthetic/orthotic device use, and adaptive equipment.
* Fabricate splints as needed.
** Qualifications**
* ** Valid California OT License
*** Current
** CPR certification**
* ** 1+ years of clinical experience
** as an Occupational Therapist
* Reliable transportation and valid driver’s license with insurance
