Your Role in our Company will be: The LVN supports the Director of Nursing, Case Management and the nursing staff in delivering healthcare by performing patient care assessments, and various medically related tasks. The LVN is a licensed nurse who works in a Home Care Setting to provide nursing services.

Responsibilities:

• Providing nurse visits to home health patients.
• Care for ill, injured, or convalescing patients by implementing plan of care initiated by RN.
• Provide basic patient care and treatment and document consistently with the plan of care.
• Communication with patients to schedule visits.
• Educate the patient on how to take care during non-visit days.
• Demonstrate knowledge of regulatory requirements.

Availability: Our patients need care 7 days a week, although you have flexibility in your scheduling, we ask that you are able to work at least two–three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).

Experience and Qualifications:

• Visiting LVN for Home Health: 1 year (Required)
• Nursing: 1 year (Required)
• Current CA LVN License (Required)
• BLS from the American Heart Association and/or the American Red Cross (Required)
• NSO Nursing License Insurance (Required)
• IV/Phlebotomy Certified (Preferred)

Work Location: City close to you, with a cluster of patients who need care for that day
