Founding Engineer

We’re hiring a Founding Engineer to join a YC-backed healthcare AI startup that is already live in hospitals and scaling rapidly.

This is a rare opportunity to become the second engineering hire outside the founders, with true ownership over core systems and meaningful influence on the technical direction of the company.

Healthcare operations are broken. Clinics are overloaded, patients wait months for care, and doctors spend valuable time gathering information instead of treating patients.

This company is building autonomous AI voice agents that fully handle patient calls, intake, insurance verification, documentation, and EHR workflows. These systems are already deployed in live clinical environments, saving 8+ minutes per patient interaction and achieving 90%+ satisfaction.

Now the team is scaling, and this hire is critical.

As a Founding Engineer, you will work directly with the founders and own systems end-to-end from day one.

• Own critical parts of the stack: AI voice agents, backend infrastructure, and EHR integrations
• Build and ship production systems quickly, including LLM pipelines and real-time services
• Work directly with clinics to prototype, deploy, and iterate on live products
• Help define engineering standards, architecture, and technical direction
• Build 0→1 systems in a fast-moving, high-trust environment

• Have 2-4 years of experience in fast-paced startups (ex-YC experience is a plus)
• Are a backend-leaning engineer comfortable working full stack when needed
• Are strong with Type Script, Node.js, AWS, and SQL
• Have experience with or strong interest in LLMs (RAG, fine-tuning, pipelines)
• Thrive in ambiguity and enjoy owning problems end-to-end
• Care about building products with real-world impact
