Heritage Senior Living is seeking an experienced and compassionate Wellness Director to lead the clinical and resident wellness operations of our community.

Reporting to the Executive Director, the Wellness Director is responsible for resident care and wellness, regulatory compliance, clinical documentation, medication management, infection prevention, and the leadership and development of our nursing and caregiver teams. This is an opportunity for a nurse who wants to combine clinical expertise, leadership, and meaningful relationships with residents and families to help create an exceptional senior living experience.

What You’ll Do Lead Resident Care & Wellness Oversee the delivery and coordination of resident care and wellness services throughout the community. Conduct new-resident interviews and assist with clinical data collection and required admission documentation. Review medical provider information and resident needs prior to move-in. Ensure the care team understands new residents' care needs, preferences, and special dietary requirements. Conduct regular resident check-ins and identify ongoing needs that support health, safety, and wellness.

Meet with residents and families to discuss wellness needs, concerns, and changes. Coordinate with physicians and other healthcare providers regarding resident health needs. Track physician appointments and follow up on recommendations and orders. Ensure changes in resident condition are appropriately evaluated, communicated, documented, and addressed. Manage Individualized Service Plans Develop and maintain Individualized Service Plans (ISPs) according to applicable requirements and community procedures.

Complete ISPs within required time frames following admission. Review and update ISPs at least every six months and following significant changes in resident condition as required. Conduct ISP meetings with residents, families, caregivers, nursing staff, and other appropriate members of the care team. Ensure resident services accurately reflect identified needs, preferences, physician orders, and changes in condition. Communicate ISP updates to employees responsible for resident care.

Monitor care delivery to help ensure services are consistent with each resident's plan. Oversee Medication Management Provide oversight of the community medication-management program. Monitor medication administration practices, documentation, storage, and related procedures. Conduct routine medication and MAR audits and ensure identified concerns are appropriately addressed. Monitor resident medication schedules and help ensure adequate medication supplies are maintained. Review and process incoming medication orders and changes.

Communicate medication and treatment changes to appropriate caregivers, CNAs, LPNs, and other clinical team members. Provide or coordinate employee education related to medication administration. Process MAR documentation and required resident summary notes. Coordinate periodic review of psychotropic medications with appropriate healthcare providers and the resident's care team. Promote safe medication practices consistent with applicable requirements and company procedures.
