Disability Services Consultant - Bachelor's

We are seeking an IRIS Consultant to support individuals in maintaining independence and self-direction through the IRIS program.

• Building trusting relationships with participants
• Developing IRIS Support and Services Plans
• Conducting regular contact and visits to ensure participant needs are met
• Assisting with understanding and maintaining IRIS eligibility
• Connecting participants with local agencies and resources
• Accurately documenting all interactions within required systems
• Possibly conducting annual functional screens
• Working independently while collaborating with a team

• Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience; or current IRIS Consultant in good standing
• Bilingual skills are a plus
• Strong commitment to participant autonomy and respectful service
• Proficiency with smartphones, tablets, and computer systems
• Knowledge of social behavior, community resources, and cultural competence
• Valid driver's license and reliable transportation
