Role
Care Coordinator for a Birth to Three Program, supporting children aged 0–3 with developmental delays or disabilities and their families.
ResponsibilitiesConduct assessments, develop and implement family-centered service plans, coordinate services, provide education and advocacy, facilitate team meetings, and ensure accurate documentation. Collaborate with families, service providers, and community resources to support child development and family needs. Maintain flexibility to adapt to client schedules and meet productivity expectations.
QualificationsBachelor's degree in Social Work, Child Development, or related field; experience working with families with special needs preferred. Valid driver's license, reliable transportation, and strong communication, organizational, and documentation skills are required. Ability to work in diverse settings and respond to crisis situations is essential.
Work EnvironmentCommunity-based, involving travel, home visits, and interaction in various environments, with moderate physical activity involved.
