and Responsibilities
- Deliver skilled physical therapy services in accordance with physician orders.
- Evaluate patient needs, create and adjust care plans to achieve therapy goals.
- Collaborate with interdisciplinary teams to set and review goals for each patient.
- Active state license as a Registered Physical Therapist.
- Current CPR certification.
- Health Certificate as required by state regulations or specific contracts.
