The Licensed Practical Nurse (LPN) is responsible for providing nursing care within the scope of practice for a LPN. In addition to hourly rate, part-time positions will receive $2.55 per hour in lieu of fringe rate. Recruitment & Retention, part-time Neighbor's positions receive $1.00 additional per hour, the first of the month, following a 30 day wait.
Primary ResponsibilitiesThe following duties are primarily performed and which are essential for this position. Employees are expected to be able to perform each of these job duties satisfactorily and successfully to be qualified for the position. Other duties may be required and assigned.
- Administers and documents medications as ordered and under guidelines of LPN licensure.
- Counts narcotics with oncoming shifts.
- Provides routine patient care and treatments including wound care, blood pressure checks, catheterizations, tube feedings, etc. following guidelines and other regulation.
- Takes physician orders over the phone.
- Completes documentation and other paperwork including patient charts, assessments, orders, etc.
- Gathers data for admissions and ongoing care of residents.
- Provides assistance with transferring and transporting patients.
- Follows universal precautions.
- Responds to resident lights and calls (e.g. wander alarms and motion alarms).
- Recognizes and reports sign and/or symptoms of significant resident health changes, resident abuse or misappropriations and reports changes to the RN.
- Acts as a team leader to Certified Nursing Assistants.
- Responds to emergencies in accordance with nursing home guidelines which may include but is not limited to performing CPR, transporting patients, or operating equipment such as a fire extinguisher.
- Attends staff meetings, in-services or other meetings.
- Performs other duties of a comparable level/type, as required.
EDUCATION AND/OR EXPERIENCE REQUIREMENTS
- LPN Diploma from an accredited school of nursing.
- Requires a minimum of one (1) year previous nurse or health related experience.
- Current licensure in the State of WI as a Licensed Practical Nurse
- CPR Certification

QUALIFICATIONS
In order to perform the duties of the position (listed above) the following knowledge, skills, and abilities are essential for the Licensed Practical Nurse to possess.
- Knowledge of the fundamentals of nursing care practices, techniques, and procedures in providing nursing care and treatments for geriatric, disabled, or impaired residents.
- Knowledge of relevant laws, rules and regulations pertaining to the delivery of nursing services to residents.
- Knowledge and fundamentals of disease processes, issues and problems associated with aging and long-term care needs of residents.
