University Hospitals is seeking an Advanced Practice Provider (FNP or PA) for the Family Medicine team at Lake County Family Medicine in Mentor, OH. The schedule is Monday through Friday, 7:30 am to 4:30 pm, with 36 hours of patient care and 4 hours of administrative time.
APP outpatient experience is preferred. The APP will provide quality care, perform assessments, determine diagnoses, and develop plans of care aligned with care Paths and UH bylaws.
